Care Manager, Dallas Texas

Key Responsibilities 

​Creating and updating personalized care plans for patients.

​Collaborating with patients' families, friends, and social supports in developing treatment plans.

Identifying and recruiting high-quality healthcare providers.

Link patients to social services programs and entitlements such as transportation assistance and translation services.

Conduct regular follow-ups with patients to evaluate progress, promote continuity of care, and ensure improved health outcomes.

Maintain and secure records of patients for the company.

Acts as a focus and resource for the organization’s privacy, security, and breach notification.

Oversees implements and monitors the security requirements levied by Federal and State Rules and Regulations.

Drafting documentation, processes and workflows in Microsoft office applications. Reviewing and approving requests for disclosure of PHI for purposes of legal proceedings such as lawsuits or government investigations.

Ensuring that security measures are in place to prevent unauthorized access to PHI.

 

Travel Requirement:

Travel to the company headquarters in Dallas, Texas once a month.

Work Schedule:

40 hr/wk, Monday through Friday, 8:30 a.m. to 5:00 p.m.

 

Requirements:

Bachelor’s Degree in science, health, business, or related, or foreign equivalent.

Two years of experience in the healthcare industry.

Registered Nurse (RN)

The Registered Nurse (RN) will provide professional nursing care to patients in their homes, adhering to the policies and procedures of Benaiah Health Care. The RN will perform clinical assessments, develop care plans, administer medications, and collaborate with other healthcare professionals to deliver exceptional patient care. Additionally, the RN will be responsible for recruiting, hiring, and supervising Licensed Vocational Nurses (LVNs), ensuring they are properly trained and integrated into the home health team. This role requires strong clinical skills, effective communication, and the ability to work independently. The position is on a contract basis with flexible hours, including on-call and as-needed shifts.

Key Responsibilities:

Conduct comprehensive patient assessments to determine healthcare needs and develop individualized care plans.

Administer medications and treatments as prescribed by the physician, ensuring compliance with medical protocols and guidelines.

Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.

Provide education and support to patients and their families regarding health conditions, treatment plans, and home care techniques.

Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care and ensure a multidisciplinary approach.

Maintain accurate and timely documentation of patient care, assessments, and interventions in electronic health records (EHR).

Respond to medical emergencies and provide appropriate interventions within the scope of practice.

Ensure compliance with all federal, state, and local regulations and standards of care.

Participate in ongoing education and training to stay current with best practices in home health care.

Recruit, hire, and supervise Licensed Vocational Nurses (LVNs), providing orientation, training, and ongoing support to ensure high standards of care.
Evaluate LVN performance and provide feedback, coaching, and professional development opportunities.


Qualifications:

Current and valid Registered Nurse (RN) license in the state of Texas.
Bachelor of Science in Nursing (BSN) preferred, Associate Degree in Nursing (ADN) required.
Minimum of [2-3] years of nursing experience, preferably in home health, critical care, or medical-surgical settings.
Experience in recruiting, hiring, and supervising nursing staff is highly desirable.
Current CPR certification.
Proficiency in using electronic health records (EHR) and other healthcare technology.
Excellent communication, interpersonal, and organizational skills.
Ability to work independently and manage multiple priorities in a dynamic environment.
Valid driver’s license and reliable transportation to travel to patient homes.


Physical Requirements:

Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.

Capacity to handle exposure to various environmental conditions in patient homes.

Licensed Vocational Nurse (LVN)

Key Responsibilities:

Provide direct patient care under the supervision of registered nurses and physicians.

Administer medications, monitor vital signs, and maintain accurate patient records.

Certified Nursing Assistant (CNA)

Benaiah Healthcare Services is seeking a dedicated and compassionate Certified Nursing Assistant (CNA) to join our healthcare team. The ideal candidate will possess strong caregiving skills, a patient-centered approach, and the ability to work collaboratively with our healthcare professionals to enhance the well-being of our patients.

 

Responsibilities:

Help clients take prescribed medication.

​Assist clients with ambulation and mobility around the house or outside (doctor’s appointments, walks etc.)

Assist clients with personal care and hygiene.

Help clients with physical therapy exercises.

Plan and prepare meals with assistance from the clients (when they are able).

Do the client’s shopping or accompany them when they shop.

Perform light housekeeping duties that clients can’t complete on their own.

Be a pleasant and supportive companion.

Report any unusual incidents.

Act quickly and responsibly in cases of emergency.

 

Skills

Proven experience as a caregiver.

Excellent knowledge of emergency response and first aid (CPR).

Knowledge of housekeeping activities and cooking with attention to dietary constraints.
